Citation: On this date at approximately 0745 hours while performing an unscheduled 03B02 procedure, the following non-compliance was observed: The Raw Not Ground/ Ground CCP Monitoring Log for processing had not been verified the weeks of 09/12/2010, 10/17/2010 and 10/24/2010. The Pre-operation/Operation Log for processing had not been verified the weeks of 10/18/2010 and 10-25-2010. The establishment shall maintain records documenting verification procedures and results. I notified Mr. verbally and with this written notice. This document serves as written notification that failure to comply with regulatory requirements could result in additional regulatory or administrative action.

Regulation:

417.5(a)(3) Records documenting the monitoring of CCP’s and their critical limits, including the recording of actual times, temperatures, or other quantifiable values, as prescribed in the establishment’s HACCP plan; the calibration of process-monitoring instruments; corrective actions, including all actions taken in response to a deviation; verification procedures and results; product code(s), product name or identity, or slaughter production lot. Each of these records shall include the date the record was made.
