Citation: On this date at approximately 0930 hours while performing an unscheduled 03A01 procedure, the following non-compliance was observed; The last signature indicating the completion of the establishment's required annual reassessment of the HACCP plan is dated 05/11/2009. Every establishment is required to reassess the adequacy of its HACCP plan at least annually. Entries clearly indicate modifications being made to the plan as late as 09/23/2001, however, there is no documentation that reassessment occurred within the one year time frame. I notified Mr. verbally and with this written notice. This document serves as written notification that failure to comply with regulatory requirements could result in additional regulatory or administrative action.
417.2(d)(2) The HACCP plan shall be dated and signed: (i) Upon initial acceptance; (ii) Upon any modification; and (iii) At least annually, upon reassessment, as required under §417.4(a)(3) of this part.
