Citation: On this date at approximately 0930 hours while performing an unscheduled 03A01 procedure, the following non-compliance was observed; The last signature indicating the completion of the establishment's required annual reassessment of the HACCP plan is dated 05/11/2009. Every establishment is required to reassess the adequacy of its HACCP plan at least annually. Entries clearly indicate modifications being made to the plan as late as 09/23/2001, however, there is no documentation that reassessment occurred within the one year time frame. I notified Mr. verbally and with this written notice. This document serves as written notification that failure to comply with regulatory requirements could result in additional regulatory or administrative action.

Regulation:

417.2(d)(2) The HACCP plan shall be dated and signed: (i) Upon initial acceptance; (ii) Upon any modification; and (iii) At least annually, upon reassessment, as required under 417.4(a)(3) of this part.
