Citation: On 4/22/2011, at approximately 0800 hours, while performing an unscheduled 03J02 procedure, the following non-compliance was observed, while reviewing the slaughter monitoring log: No monitoring of the Mix on 4/4/2011 & 3/21/2011. Records indicate that Beef was slaughtered on those dates and the (CCP 1B) was applied to the carcasses; however records indicate that the mixing of was not monitored. Also, records show that a monthly observation of the CCP's was not performed for the month of March 2011. I notified Mr. of these non-compliances. This document serves as a written notice that your failure to comply with regulatory requirements could result in additional regulatory or administrative action.

Regulation:

417.5(a)(3) Records documenting the monitoring of CCP’s and their critical limits, including the recording of actual times, temperatures, or other quantifiable values, as prescribed in the establishment’s HACCP plan; the calibration of process-monitoring instruments; corrective actions, including all actions taken in response to a deviation; verification procedures and results; product code(s), product name or identity, or slaughter production lot. Each of these records shall include the date the record was made.
