Citation: On October 25, 2011 I visited your establishment and reviewed your record, the Slaughter Inspection Report (DOC. # JHM-FORM-003), dated 10/25/11. Upon reviewing the record at approximately 1400 hours, the only documentation was [redacted] sheep slaughtered with the two zero tolerance checks. On October 26, 2011 while visiting your establishment to perform a records review, I again reviewed this same document and found the following noncompliance: On this record it was now documented that [redacted] goat and [redacted] lamb were slaughtered. The times of checks for the goats are documented as being performed at 11:37, 12:10, and 2:20. This would be noncompliant as the 11:37 and 12:10 checks were not documented on this record, at the time the event occurred.[newline][newline]A previous noncompliance was issued on August 31, 2011 (Noncompliance #XVH1811083231N) for the same issue.
417.5(b) Each entry on a record maintained under the HACCP plan shall be made at the time the specific event occurs and include the date and time recorded, and shall be signed or initialed by the establishment employee making the entry.
