Citation: At 1130 hours during a scheduled 03G02 procedure the following noncompliance was noted. 1) Hot dog products were produced on Monday (05/09/11). I confirmed through conversation with , Haccp/QC Manager that monitoring and direct observation verification for both CCP-B2 (Cooking) and CCP-B6 (Chilling/Cooling) were performed by . 2) On the Weekly Verification Log monitoring for CCP-B2 (Cooking) was performed at 1015 hours, the corresponding direct observation of the monitoring activity was documented at 1100 hours. For CCP-B2 under verification states that "[redacted]". 3) On this same record monitoring for CCP-B6 (Chilling/Cooling) was performed at 1100 hours and direct observation of the monitoring activity was documented at 1100 hours. For CCP-B6 under verification states that "[redacted]". 4) At the bottom of both CCP's the following is stated "[redacted]". These findings are in violation of 9 CFR 417.4(a), 417.4(a)(2)(ii), 417.5(b), and 417.6(a)&(b). This noncompliance is being linked to NR#25 dated 05/10/11, NR #23 dated 05/04/11 and NR #16 dated 04/07/11 as the cause is the same. The establishment is not verifying that the plan is being effectively implemented. These NR's have not been closed; the establishment elects not to document any written corrective actions, has not proffered any responses verbally, and no changes have been documented in the hazard analysis to bring them back into compliance with the regulatory requirements.

Regulation:

417.4(a)(2)(ii) Direct observations of monitoring activities and corrective actions;

417.5(b) Each entry on a record maintained under the HACCP plan shall be made at the time the specific event occurs and include the date and time recorded, and shall be signed or initialed by the establishment employee making the entry.

417.6(a) The HACCP plan in operation does not meet the requirements set forth in this part;

417.6(b) Establishment personnel are not performing tasks specified in the HACCP plan;
