Citation: While conducting a walkthrough of the facility during a FSA entrance meeting with EIAO & Dr. at approximately 11:15 am, we noticed the following 06D01 non-compliance issues. At the left hand bottom corner of the doorway leading into the inedible storage room there exists a gap approximately 3/4" wide by 2" high, which could lead to the entrance of vermin. Also in the compressor room area there is a hole approximately 1 1/2" in diameter, through the wall leading to the animal holding pens, in this hole exists a water hose that is approximately 1" in diameter, leaving a 1/4" gap around said hose which also could lead to the entrance of vermin, insects etc. was notified verbally of this non-compliance at 3:15pm

Regulation:

416.2(b)(3) Walls, floors, ceilings, doors, windows, and other outside openings must be constructed and maintained to prevent the entrance of vermin, such as flies, rats, and mice.
