Citation: At approximately 0830 hours while reviewing records performing a scheduled 03G02 procedure the following was observed. The "Cooking /Cooling Log" dated 5/4/11 documentation was provided for a lot of "Hot Sticks" lot # 5411 was produced on 5/4/11. No initials were provided for the individual recording temperatures of the cooling cycle temperatures and times were recorded in the appropriate blocks but no initials were documented for the individual performing the temperature checks. The records review block was filled in and documented that a record review was performed on this lot and all records were complete and correct this was signed by Mr. [redacted] the record review made no mention of the missing documentation. Mr. [redacted] was notified verbally and in writing of this NR of this non- compliance Reg 9 CFR 417.5(b) states Each entry on a record maintained under the HACCP plan shall be made at the time the specific event occurs and include the date and time recorded, and shall be signed or initialed by the establishment employee making the entry Reg 9 CFR 417.5(c) states Prior to shipping product, the establishment shall review the records associated with the production of that product, documented in accordance with this section, to ensure completeness, including the determination that all critical limits were met and, if appropriate, corrective actions were taken, including the proper disposition of product. Where practicable, this review shall be conducted, dated, and signed by an individual who did not produce the record(s), preferably by someone trained in accordance with 417.7 of this part, or the responsible establishment official

Regulation:

417.5(b) Each entry on a record maintained under the HACCP plan shall be made at the time the specific event occurs and include the date and time recorded, and shall be signed or initialed by the establishment employee making the entry.

417.5(c) Prior to shipping product, the establishment shall review the records associated with the production of that product, documented in accordance with this section, to ensure completeness, including the determination that all critical limits were met and, if appropriate, corrective actions were taken, including the proper disposition of product. Where practicable, this review shall be conducted, dated, and signed by an individual who did not produce the record(s), preferably by someone trained in accordance with 417.7 of this part, or the responsible establishment official.
