Violation: 417.4(a)(2)(ii), 417.4(a)(2)(iii), 417.4(a)(3), 417.5(a)(3)

Citation: On April 26, 2012 at approximately 0900 while performing a directed Fully Cooked/Not shelf stable HACCP task the following noncompliance was identified. I reviewed the records associated with CCP 1B(Cook) and CCP 2B(Chill) titled Ring Bologna Cooking and Chilling Log dated 2-6-12 through 4-23-12. I observed for the month of March 2012, on 3-26-12 the establishment recorded a direct observation at 10:45am for CCP 1B, however no direct observations for CCP 2B were recorded for the month of March. The establishment's Fully Cooked/Not Shelf Stable HACCP plan dated 6-17-10 states for CCP 2B "[redacted]  . While reviewing the same records I found only one records review verification for the month of March with no reference to which CCP the records review verification is associated with. According to the establishments same HACCP plan, Upon review of the Fully Cooked/Not Shelf Stable HACCP plan, the annual reassessment had not been conducted for the 2011 year. This is failure to comply with 9CFR 417.4(a)(3). Owner, Mr. Mike Biser, was notified of the above noncompliance.


417.4(a)(2)(ii) Direct observations of monitoring activities and corrective actions;

417.4(a)(2)(iii) The review of records generated and maintained in accordance with § 417.5(a)(3) of this part.

417.4(a)(3) Reassessment of the HACCP plan. Every establishment shall reassess the adequacy of the HACCP plan at least annually and whenever any changes occur that could affect the hazard analysis or alter the HACCP plan. Such changes may include, but are not limited to, changes in: raw materials or source of raw materials; product formulation; slaughter or processing methods or systems; production volume; personnel; packaging; finished product distribution systems; or, the intended use or consumers of the finished product. The reassessment shall be performed by an individual trained in accordance with § 417.7 of this part. The HACCP plan shall be modified immediately whenever a reassessment reveals that the plan no longer meets the requirements of § 417.2(c) of this part.

417.5(a)(3) Records documenting the monitoring of CCP’s and their critical limits, including the recording of actual times, temperatures, or other quantifiable values, as prescribed in the establishment’s HACCP plan; the calibration of process-monitoring instruments; corrective actions, including all actions taken in response to a deviation; verification procedures and results; product code(s), product name or identity, or slaughter production lot. Each of these records shall include the date the record was made.
