Code: 06D01
Violation: 416.1, 416.3(c), 416.4(b)

Citation: At 0700 hours on the 23rd of February; after plant's preoperational sanitation inspection, while performing preoperational sanitation verification, the following was observed: *two white lugs on the rack had no identity of use in the eviscerating area; *organic stains on the cooler door 3 facing inside the cooler; *green to dark green mildew forming on the right corner floor inside the cooler 3 at the entrance; [redacted] Jr., Operations Manager, was informed of the noncompliance. Subsequently, the lugs were labeled; the cooler door and floor were cleaned. The establishment's preoperational sanitation monitoring log for this date has no documentation reflecting to the aforementioned findings. This is a failure of 9 CFR 416.1, 416.3(c), and 416.4(b). Past NR 0002-2011-17299, dated 1/6/11, is the trend of similar cause.
