Citation: On November 17, 2010, at approximately 12:30 pm while performing PBIS task 03J01 verification requirement and the recordkeeping component, the following noncompliances were observed. While performing a review of the records for CCP 2 & CCP 3, the form called [redacted] for the two interventions the record shows the monitors initials, date and time of event. However, there is no time of event for the employees who perform the ongoing verification results for direct observation and record review. Mr. [redacted] was verbally notified of the above noncompliance. This is a violation of 9 CFR 417.5 (a) "The establishment shall maintain the following records documenting the establishment's HACCP plan: (3) Records documenting the monitoring of CCP's and their critical limits, including the recording of actual times, temperatures, or other quantifiable values, as prescribed in the establishment's HACCP plan; the calibration of process-monitoring instruments; corrective actions, including all actions taken in response to a deviation; verification procedures and results; product code(s), product name or identity, or slaughter production lot. Each of these records shall include the date the record was made." 417.5(b) "Each entry on a record maintained under the HACCP plan shall be made at the time the specific event occurs and include the date and time recorded, and shall be signed or initialed by the establishment employee making the entry." I then reviewed the NR file and found no recent noncompliance report of this nature.

417.5(a)(3) Records documenting the monitoring of CCP’s and their critical limits, including the recording of actual times, temperatures, or other quantifiable values, as prescribed in the establishment’s HACCP plan; the calibration of process-monitoring instruments; corrective actions, including all actions taken in response to a deviation; verification procedures and results; product code(s), product name or identity, or slaughter production lot. Each of these records shall include the date the record was made.

417.5(b) Each entry on a record maintained under the HACCP plan shall be made at the time the specific event occurs and include the date and time recorded, and shall be signed or initialed by the establishment employee making the entry.
