This portion of the form also has room for notations of corrective actions taken for deficiencies observed. This portion of the form was completed for checks and observations for the pre-operational sanitation portion only. The portion of this form that deals with the operational sanitation observations had a listing of the time performed only in the heading. The lower portion that has the check-off sheet had no notation for checks performed of the previous day (December 6, 2010) dealing with operational sanitation. There were no markings on any of the listed entries for areas or observations on the operational portion of this form.
