Citation: At approximately 0557 hours, I entered the processing area to perform a 01B02 procedure. After confirming with the monitor that the establishment has completed their pre-operational inspection; I performed my own observation. During this observation I found noncompliance and the following is what I found: The Butcher Boy Grinder held pieces of meat and build up on the left side auger blade. These findings were scattered about that one side of the auger and the largest piece of meat found was approximately a 1/2 " in size. Inside of the other Butcher Boy Grinder/ mixer; two of the containing parts held areas of rust. The rust was brownish in color and came off onto my fingertips when I rubbed across it. As I was checking the cutting boards a fly was flying around and then I observed the fly land on the cutting board normally used for slicing bellies, shoulders, etc. I then went to the pack room to continue with my observation. As soon as I entered this area I noticed another fly had landed on the cutting/packing table and a knat like insect on the outside of the dip equipment. Along the side of this cutting board was pieces of meat; as well. [redacted] and [redacted] were shown the grinder deficiencies found and was verbally notified of this noncompliance. [redacted] was also shown the knat insect and the pieces of meat left on the side of the cutting board. This verification was performed prior to the start of operations, no product was out at the time of these findings and the establishment was not permitted to start certain areas and/or equipment until the appropriate corrective actions were taken. No other additional actions were taken because the establishment began taking corrective actions immediately. At the end of this procedure; I reviewed the monitors pre-operational records. The records showed these pieces of equipment to be acceptable.


416.13(c) Each official establishment shall monitor daily the implementation of the procedures in the Sanitation SOPís.

416.2(a) Grounds and pest control. The grounds about an establishment must be maintained to prevent conditions that could lead to insanitary conditions, adulteration of product, or interfere with inspection by FSIS program employees. Establishments must have in place a pest management program to prevent the harborage and breeding of pests on the grounds and within establishment facilities. Pest control substances used must be safe and effective under the conditions of use and not be applied or stored in a manner that will result in the adulteration of product or the creation of insanitary conditions.

416.3(a) Equipment and utensils used for processing or otherwise handling edible product or ingredients must be of such material and construction to facilitate thorough cleaning and to ensure that their use will not cause the adulteration of product during processing, handling, or storage. Equipment and utensils must be maintained in sanitary condition so as not to adulterate product.

416.4(a) All food-contact surfaces, including food-contact surfaces of utensils and equipment, must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
