Citation: At approximately 0600 hours; I began scheduled procedure 01B02 in the processing and pack room areas. This procedure was prior to the start of production and after the monitor completed his inspection. During my observation I found noncompliance and the following was observed: - In the pack room; one of the large knives was found with a piece of meat at the rear end of the stainless steel blade ( near the handle). This finding was approximately 1/2" in size. In the processing room the following was observed: - A dead fly was lying atop of the stainless steel table by the sausage stuffer machine. - Inside of the ' Butcher Boy' Grinder was a fly on one of the auger blades. This fly was still alive. - Inside of the ' Butcher Boy' Grinder/ Mixer was a buildup of product residues, under the worm arm. This finding was against the right side wall and covered approximately a 2" diameter of this side wall. Several small meat particles were found below it; as well. U.S. reject tag # B36279398 was applied to the 'Butcher Boy' grinders. Mr. [redacted] was shown most of these findings and verbally notified of this noncompliance. Corrective actions were immediately taken on the other deficiencies found. Approximately 0620 hours Mr. [redacted] was also verbally notified of this noncompliance. The regulatory control action taken remained in place and was not relinquished until the appropriate corrective action's were taken and verified.


416.13(c) Each official establishment shall monitor daily the implementation of the procedures in the Sanitation SOPís.

416.2(a) Grounds and pest control. The grounds about an establishment must be maintained to prevent conditions that could lead to insanitary conditions, adulteration of product, or interfere with inspection by FSIS program employees. Establishments must have in place a pest management program to prevent the harborage and breeding of pests on the grounds and within establishment facilities. Pest control substances used must be safe and effective under the conditions of use and not be applied or stored in a manner that will result in the adulteration of product or the creation of insanitary conditions.

416.4(a) All food-contact surfaces, including food-contact surfaces of utensils and equipment, must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
