Citation: At 0600 hrs on the 2nd of November, after the establishment concluded their preoperational sanitation inspection, while performimg preoperational sanitation verification, I observed the following noncompliance : Processing Room * organic residue from the previous days' operation on the patty machine transport belt (food-contact-surface); * organic residue from the previous days' operation on the slicer transport belt; specifically, in a seem that runs across the width of the belt (food-contact-surface); * organic residue on the scale stand frame adjacent to the flaker; * organic residue on the exterior bottom a grey bucket, marked edible, stored on the sink adjacent to grinder #2; * organic matter and fuzzy mold on the breading machine; specifically, on the top section of the feed worm (food-contact-surface); * seasoning from the previous days' operation on the stainless steel packing table, scaleframe, wall and floor adjacent to the freezer; * accumulation of organ ic residue on a plastic foot stool adjacent to the slicer; * accumulation of organic residue on a stainless steel foot stand adjacent to the freezer; * organic residue on the lower frame of two stainless steel tables adjacent to the slicer. [redacted] ,was advised of the aforementioned noncompliance. The plant's preoperational sanitation monitoring log has no documentation reflect ing the insanitary conditions.I n addition, the plant's Sanitation SOP, preoperational sanitation section states that the * .This is a failure of 9 CFR 416.13(a); 416.4(b);and 416.4(a) . Consequently, official control action initiated and US Reject Tag #87619082 applied to the processing room. Equipment was cleaned, sanitized, inspected and released . Repetitive NRs: Management's unwillingness/inability to properly implement the written procedures in the SSOP, and subsequent failure to prevent contamination of food-contact-surfaces prior to the start of operations, has resulted in a trend of noncompliance identified as #6.NR #MDG4306103827/2, issued on 10/27/2011, is identified as trend #5. Trends of noncompliance are identified as having the same/similar root cause.

Regulation:

416.13(a) Each official establishment shall conduct the pre-operational procedures in the Sanitation SOPís before the start of operations.

416.4(a) All food-contact surfaces, including food-contact surfaces of utensils and equipment, must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
