Citation: At 0600 hrs on the 29th of November, after the plant concluded their preoperational sanitation inspection, while performing preoperational sanitation verification, I observed the following noncompliance: Processing Room * several small pieces of organic matter inside a large stainless steel wheel tub adjacent to the flaker (food-contact-surface); * mold and organic debris inside a box containing papers, cryovaced meat, pencils, pens, and labels, stored on a storage rack adjacent to the retail store entrance: * dry organic matter under the lid on the vacuum machine was advised of the aforementioned noncompliance. The plant's preoperational sanitation monitoring log has no documentation reflecting the insanitary conditions. In addition, the plant's Sanitation SOP, preoperational sanitation section states that the [redacted] This is a failure of 9 CFR 416.4(a); 416.4(b); and 416.13(a). Consequently, official control action was initiated and US Reject Tag #87619083 was applied to the stainless steel tub. The box stored on the rack was removed from the room immediately. The tub was cleaned, sanitized, inspected and released. Repetitive NRs: Management's unwillingness/inability to properly implement the written procedures in the SSOP, and subsequent failure to prevent contamination of food-contact-surfaces prior to the start of operations, has resulted in a trend of noncompliance. This NR is associated with NR #MDG2106112922, dated 11/22/2011. Trends of noncompliance are identified as having the same/similar root cause.

Regulation:

416.4(a) All food-contact surfaces, including food-contact surfaces of utensils and equipment, must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.

416.13(a) Each official establishment shall conduct the pre-operational procedures in the Sanitation SOPís before the start of operations.

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
