Citation: On 30 August 2010, at approximately 1140 hrs, whilst performing the review and observation component of the scheduled heat-treated, not fully cooked, not shelf stable procedure, the following unscheduled ISP procedure code 06D01 (Facilities & Equipment) was performed and the following noncompliances were identified in the heat-treated blood processing room: (1) The paper towel dispenser to the immediate left of the hand wash sink in this area was discovered to be completely devoid of paper towels; (2) The aforementioned hand wash sink was discovered to contain only cold running water. I took immediate regulatory control action and affixed US Rejected/US Retained tag number B38869223 on top of the soap dispenser. [redacted] QC, was notified of the noncompliance at the time of occurrence. The establishment replaced the paper towels and turned on the hot water valve by 1155 hrs, at which time I reviewed their efforts, deemed them to be satisfactory, and removed the US Rejected/US Retained tag and released the area for further production. The establishment is not in compliance with the following regulations: 9 CFR 416.1, which states the following: "General rules. Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated."; 9 CFR 416.2(h)(2), which states the following: "Lavatories with running hot and cold water, soap, and towels, must be placed in or near toilet and urinal rooms and at such other places in the establishment as necessary to ensure cleanliness of all persons handling any product.".

Regulation:

416.1 Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.

416.2(h)(2) Dressing rooms, lavatories, and toilets. Lavatories with running hot and cold water, soap, and towels, must be placed in or near toilet and urinal rooms and at such other places in the establishment as necessary to ensure cleanliness of all persons handling any product.
