Citation: While reviewing your HACCP records, Form 004 ,005, 006 and 007, which record your titration of PAA (acid) antimicrobial treatment of raw product, I noticed that you are not titrating at the rate given in your HACCP Plans 001, 002, 003 and 004. Your plan [redacted] When I reviewed the recordings for Sept 17, 2010, I see that the titrations that is to occur before the start of operations does not occur. These titrations and the PAA application checks are listed as CCP1B. 417.2(c)(4) states [redacted]." Also your Form 005 states [redacted]. When I reviewed several copies of your Form 010 and 011 (Pre-shipment Review forms), I noticed that the QC person signing the forms is the same QC person that recorded the Monitoring results. Your 4 HACCP Plans all state (on page 2) that "[redacted] .....
