Citation: On May 15, 2012 at approximately 23:30 hours, I performed a Fully Cooked- Not Shelf Stable HACCP Verification Task (Recordkeeping Component). To perform this procedure, I verified that the monitoring of all critical control points were performed at the specified frequency, the critical limits were met, all verification activities were completed including; all product testing, records review, direct observations, equipment calibrations, corrective actions and the pre-shipment review was completed as outlined in the establishment's Fully Cooked- Not Shelf Stable (Cooked Cured Sausage) HACCP Plan, dated: 1/13/12. As I reviewed the establishment's HACCP records for the production date of: 5/11/12, I observed the following noncompliances:I observed on the establishment's HACCP monitoring record CCP [redacted]), the 4:40pm documented monitoring of the critical control point for product # 3301 (Deluxe Grill Franks 8-1), failed to include the plant designated monitor's initials/ signature. After further review of this record, it was also observed that the CCP Direct Observation Verification was performed at 2:30pm with NO documented "Result" and with NO documented "Date"; and the CCP Record Review Verification was performed only including a documented date and signature, but with NO documented "Result" and with NO documented "Time" of when this verification activity was performed. The individual who performed the record review verification failed to identify that the monitoring entry was not signed or initialed by the establishment employee making the entry. The Pre-Shipment Review for the aforementioned product was performed on 5/11/12 by plant designee. Further review of the establishment's HACCP Records from previous day's production's indicate that the establishment has failed to comply with the Meat and Poultry Regulatory Requirements of 9 CFR 417.5(b) and 417.5(a)(3), specifically in regards to HACCP Verification Records (Direct Observations and Record Reviews). Mr. FSQA Supervisor, was notified verbally and in writing with this noncompliance record of the establishment's failure to meet the aforementioned regulatory requirements.No NR's were found documented with similar cause within the previous 90 days.
417.5(a)(3) Records documenting the monitoring of CCP’s and their critical limits, including the recording of actual times, temperatures, or other quantifiable values, as prescribed in the establishment’s HACCP plan; the calibration of process-monitoring instruments; corrective actions, including all actions taken in response to a deviation; verification procedures and results; product code(s), product name or identity, or slaughter production lot. Each of these records shall include the date the record was made.
417.5(b) Each entry on a record maintained under the HACCP plan shall be made at the time the specific event occurs and include the date and time recorded, and shall be signed or initialed by the establishment employee making the entry.
