Citation: At approximately 0715 hours, while performing an 06D01 procedure, I observed the following noncompliances. 1. Mold on the walls and door of the men's bathroom as well as standing water approximately 1/4 inch deep on the floor of the men's room. 2. On one of the shelves in the laundry room there were numerous moldy gloves and sleeves. The lab coats had a musty smell. 3. In the chemical room, there was mold on the ceiling tiles, peeling paint on the walls, and mold on the stored helmets, boots, rain suit and boxes which were on the shelf. A moldy box, on the pallet next to the wall was filled with green scrub pads, gloves, dirty hairnets, garbage and other miscellaneous items. 4. Rusty peeling paint was noticed on the door around the window frame of the door leading to the processing room. 5. In the hallway, cobwebs and dead bugs were hanging from the corners of the ceiling. Mr. HACCP Manager was verbally notified of the non-compliances. This serves as notification that you are in noncompliance of 9CFR416.2(b)(2) and 416.4(b).


416.2(b)(2) Walls, floors, and ceilings within establishments must be built of durable materials impervious to moisture and be cleaned and sanitized as necessary to prevent adulteration of product or the creation of insanitary conditions.

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
