Citation: At approximately 0915 hours, I began reviewing records dated 8/29/2011for ground turkey produced under the Raw Ground HACCP plan in order to complete an 03B02 procedure. As part of a scheduled 03B02 procedure, yesterday, 8/29/2011, at approximately 1315 hours, I accompanied Ms. [redacted] QC, to observe her perform direct observation of the monitoring procedure for ground turkey produced under the Raw Ground HACCP plan. The monitoring of the temperature for CCP 1B at that time was [redacted] degrees Fahrenheit which was above the critical limit of  [redacted]degrees F. This was observed during the verification procedure of direct observation. Review of the record titled " Temperature Monitoring Log HACCP CCP #" shows that there was no entry around the time of 1315 hours showing that product had been monitored and had not met the critical limit. In addition, there was no entry for the direct observation that had been performed at that time. It was also noted that the record review for CCP1B had not performed at the frequency stated in the HACCP plan as the entry used to document record review as required by 9CFR417.4 was blank. The establishments HACCP plan states that " [redacted] ." Review of the pre-shipment record shows that it was performed on 8/29/2011. Additionally, review of the corrective actions shows that there is no time documented for the corrective actions taken or for the direct observation of the corrective actions. As part of the establishment's answer to identifying the cause of the deviation, they just state that " Identified during CCP check." The establishment does not identify the cause of the deviation. Mr. [redacted] was shown and verbally informed of the non-compliances. The establishment is not complying with 9CFR 417.4(a) (2)(iii) and 417.5(b).


417.4(a)(2)(iii) The review of records generated and maintained in accordance with 417.5(a)(3) of this part.

417.5(b) Each entry on a record maintained under the HACCP plan shall be made at the time the specific event occurs and include the date and time recorded, and shall be signed or initialed by the establishment employee making the entry.
