Violation: 416.2(d), 416.4(d)

Citation: At approximately 0525 hours, (CSI) and I performed a 06D01 (facility) task in the box room. I asked (HACCP/ QA Supervisor), to accompany us on a tour of the box room. We observed the following noncompliances: (1) Five areas of the box room floor were wet, 3 of these areas had large wet spots. (2) Two areas were plastic hung had not been changed as needed. The plastic was dusty and dirty. (3) UFM's were observed on a couple of skids of boxes. (4) A blue barrel had stagnant water in it. We don't know how long it was setting like that. In the salt room area, old salt was observed on the floor at the chute area. The above noncompliances are a violation of 9CFR 416.4(d) and your SPS program. At approximately 0805 hours, I observed the following noncompliance in the H2 cooler. (1) There were 5 skids of product that had an accumulation of ice/frost on top of the boxes. Chunks of ice and UFM's also were observed on these boxes. I tagged all 5 skids with USDA tag B37853315. This product will be reinspected before the removal of USDA tag. (2) Ice/frost was observed on the cooling unit and the yellow post. (3) Ice/frost was observed along the entire rack area at the wall area. The above is a violation of 9CFR 416.2(d). (Food Safety Director) and (HACCP/QA Supervisor) were informed of these noncompliances.This is written notification that your failure to comply with regulatory requirements could result in additional regulatory or administrative action.


416.2(d) Ventilation. Ventilation adequate to control odors, vapors, and condensation to the extent necessary to prevent adulteration of product and the creation of insanitary conditions must be provided.

416.4(d) Product must be protected from adulteration during processing, handling, storage, loading, and unloading at and during transportation from official establishments.
