Citation: On August 13th, 2010 as I was walking to the box room, I entered the stairwell off of the Cut-up department and found peeling paint on the underside of the steps, a green color growth on the wall, a stainless steel table of some kind covered in rust flakes and dirt falling from the underside of the steps, a large size hole in the bottom step approximately 8 to 10 inches across and 4 to 6 inches wide from the step rotting away, the door frame leading to the outside of the building near the box room elevator is rotting away on the left side of the frame about 6 inches of the frame is gone, & I saw a few spiders and cobwebs within the same area. I informed, QA Manager of my findings, area was tagged with USDA Rejected tag # B39-801577. SSOP paperwork for the Box Room that the stairs to the boc room are acceptable.


416.2(b)(2) Walls, floors, and ceilings within establishments must be built of durable materials impervious to moisture and be cleaned and sanitized as necessary to prevent adulteration of product or the creation of insanitary conditions.

416.2(b)(3) Walls, floors, ceilings, doors, windows, and other outside openings must be constructed and maintained to prevent the entrance of vermin, such as flies, rats, and mice.
