Citation: This morning while CSI was checking the box room, at approxitamly 05:40 it was observed that boxes were leaned against the wall , and a spider in the boxes. At this time a USDA Rejected tag, No # B40197826 was applied, and was notified. The box room was cleaned, all affected boxes were destroyed, and the spider disposed of. Inspection Personal removed the tag, and released the box room to production at 06:40. At 05:50 CSI noticed in the employees locker room where maintenance had hung plastic because of a ceiling/ pipe leak, the plastic had fallen down, and brown water was leaking all over the floor. At this time I again notified Maintenance personal of this, and taped the area off with caution tape, I applied USDA Reject tag No. # B39801609 to the area, he replaced the plastic and cleaned, and mopped the floor. I then reinspected the area, and removed the tag at 06:20.CSI found water dripping from a steam pipe above the Shrink Tunnel ,at this time he again notified Further Processing Manager, who notified maintance ..CSI applied USDA Reject Tag No.# B40197827 . at 07:00 the pipe was repaired,, the effected area, was reinspected and released to production at 07:29. This establishment has received both oral and written notice of these noncompliance's,
416.2(b)(3) Walls, floors, ceilings, doors, windows, and other outside openings must be constructed and maintained to prevent the entrance of vermin, such as flies, rats, and mice.
416.2(c) Lighting of good quality and sufficient intensity to ensure that sanitary conditions are maintained and that product is not adulterated must be provided in areas where food is processed, handled, stored, or examined; where equipment and utensils are cleaned; and in hand-washing areas, dressing and locker rooms, and toilets.
