Citation: On July 20th, 2011 at approximately 0855 hours, I performed a presentation verification (ISP Code 06D01) at the inspection station on the West evisceration line, at a line speed of [redacted] birds per minute (bpm). Presentation failed with a 10 bird total nonconformance weight of [redacted]. This failure consisted of 1 contamination on viscera (6 nonconformance factors), 2 viscera on shackle (8 nonconformance factors each), & 2 contamination inside bird (6 nonconformance factors each). I immediately informed 1st Processing Super. Mr. who notified QA, of the establishments failure to meet regulatory requirements. QA performed retests starting at 0913 hours and failed the line speed was reduced to [redacted] bpm, QA retest at 930 hours failed, line speed reduced to [redacted] bpm, QA retest failed at 0952 hours, line speed reduced to [redacted] bpm. At that point Mr. informed USDA & QA that the line speed would remain at [redacted] bpm until the lot was completed. Establishment management has received oral & written notification regarding this noncompliance. Failure to comply with regulatory requirements could result in additional enforcement actions as described in 9 CFR 500. Past Similar NRs - Previous Ineffective Plant Actions: Plant has not responded to NR # 0096 yet. NR: 96-2011 dated 7/13/2011


381.76(b) Presentation: Each inspector shall be flanked by an establishment employee assigned to be the inspector’s helper. The one inspector on the SIS–1 line shall be presented every bird. Each inspector on the SIS–2 line shall be presented every other bird on the line. An establishment employee shall present each bird to the inspector properly eviscerated with the back side toward the inspector and the viscera uniformly trailing or leading. Each inspector shall inspect the inside, viscera, and outside of all birds presented.
