Citation: On August 16th, 2011 at approximately 0520 hours, I performed a presentation verification (ISP Code 06D02) at the second inspection station on the West evisceration line, at a line speed of [redacted] birds per minute (bpm). Presentation failed with a 10 bird nonconformance weight of [redacted], plus 4 occurrences of one error. This failure consisted of 1 viscera below wing (12 nonconformance factors each), 1 viscera on shackle (6 nonconformance factors each), 1 opening cut (1 nonconformance factors each), 4 contamination inside bird (6 nonconformance factors each). This failure also is an immediate line reduction of 10 %. I notified Evisceration Supervisor, who notified QA Tech , of the establishments failure to meet regulatory requirements. QA performed a presentation retest at 0526 hours and failed, follow up retests also failed which lead to the line speed being reduced to [redacted] bpm at 0840 hours, which upon USDA notified that the line speed would remain at [redacted] bpm until a change of lot when retests would resume. Establishment management has received oral and written notification regarding this noncompliance. Failure to comply with regulatory requirements could result in additional enforcement action as described in 9 C.F.R. 500. Past Similar NRs - Previous Ineffective Plant Actions: Plant has not responded to NR # 110 as of yet. NR: 110-2011 dated 8/9/2011

Regulation:

381.76(b) Presentation: Each inspector shall be flanked by an establishment employee assigned to be the inspector’s helper. The one inspector on the SIS–1 line shall be presented every bird. Each inspector on the SIS–2 line shall be presented every other bird on the line. An establishment employee shall present each bird to the inspector properly eviscerated with the back side toward the inspector and the viscera uniformly trailing or leading. Each inspector shall inspect the inside, viscera, and outside of all birds presented.
