Citation: At approximately 5:00 am this morning while performing pre-op in the cut-up department I saw that the plastic racks, wheels and dividers had fat and build up on them. I tagged them with USDA reject Tag NO # B39801640. Continuing on to Breast Deboning I saw shields for the Repack machine on the floor and dirty with fat and blood from previous day's production. I tagged them with USDA reject Tag #B39801641. Also the bagger machine was filled with plastic pieces, and I tagged it with USDA reject Tag#B40170424. Then going back to Cut-up to re-inspect the plastic racks, wheels and dividers, I checked the finish product track and It had grease and fat on it. I tagged it with USDA reject Tag#B40170425. I released plastic racks at 5:24, the bagger machine at 6:14, cut-up finish product track at 6:21 and the shields for the Repack machine at 6:29. When I tagged the plastic shields for the Repack machine I noticed there already was a USDA reject tag on them, but it was about 4 shields back. It was a USDA reject tag from Inspector from yesterday. She tagged them with USDA reject #B40197839.They were washed and sanitized and put back in the electrical room on the floor and the USDA reject tag was still on them. I notified (QA) that an NR would be issed.The establishment has been notified both verbally and in writing. Failure to comply with regulatory requirements could result in additional enforcement action as described in 9CFR500. Past Similar NRs - Previous Ineffective Plant Actions: NR#122 not answered yet by establishment NR: 122-2011 dated 9/23/2011

Regulation:

416.4(a) All food-contact surfaces, including food-contact surfaces of utensils and equipment, must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
