Violation: 416.1, 416.2(e)(4), 416.4(b)

Citation: At approximately 0540 hours while performing an unscheduled 06D01 I observed several employees entering the supervisors-supply office across from the supply room. Looking in the room I observed it to be flooded, trash floating on the floor, mud every where. At this time I informed everyone they had to leave the room, as I was tagging the area. I then applied USDA Rejected Tag # B40197834 to the room. Notified showed him the room, and preceded with the tag. At 0647 hours Mr. called me to reinspect and release the office, after reinspecting the area, I removed the tag, and released the area. While walking through the plant at approximately 0816 hours I observed condensation running down the wall, and dripping from the doorway of the shipping dock cooler. At this time I notified Shipping Supervisor . I applied USDA Reject Tag # B39133649 and sealed off the doorway with caution tape caution tape. At 0847 hour I reinspected and released the cooler, removing my tag, and the caution tape. After lunch walking through thigh deboning I observed condensation on several areas of the ceiling. I informed the deboning supervisor, he had it wiped down. In evisceration I noticed plastic wrapped around some of the insulated pipes, because of leaks or condensation. There is a piece of insulation wrap falling off of a pipe along the east wall in the evisceration department. This was brought to Mr. 's attention in the weekly USDA/ Establishment meeting. At no time through these noncompliance's was there any product involved. The establishment is in violation of 9 CFR 416.1and 9 CFR 416.2(e), & 9 CFR 416.4(b). This establishment has been notified both orally and in writing of these noncompliance's. Failure to comply could result in further enforcement action as described in 9 CFR 500.


416.1 Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.

416.2(e)(4) Provide adequate floor drainage in all areas where floors are subject to flooding-type cleaning or where normal operations release or discharge water or other liquid waste on the floor;

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
