Citation: At approximately 1:40am when performing my SPS scheduled facility check I seen a Roach crawling across the lockers in the locker room on the ground floor. The Roach was on locker 406 when I noticed it; it was at least a 1/2 inch in length. When walking through the cafeteria on the ground floor I noticed the double refridgerator had yellow tape across it and a red Quality Control Tag with the word written ROACHES. The USDA grader complained this week about aunts in her office which is also on the ground floor.According to 1/12caught mice in traps 57, 60, on 2/16/12 caught on mouse in trap 60 and had 2 catches 1 in laudry room and 1 in locker room.This is a noncompliant of 416.2 Establishment grounds and facilities (a) Grounds and pest control regulation. Continued failure to meet regulatory requirements can lead to enforcement actions as described in 9CFR part 500.4.

Regulation:

416.2(a) Grounds and pest control. The grounds about an establishment must be maintained to prevent conditions that could lead to insanitary conditions, adulteration of product, or interfere with inspection by FSIS program employees. Establishments must have in place a pest management program to prevent the harborage and breeding of pests on the grounds and within establishment facilities. Pest control substances used must be safe and effective under the conditions of use and not be applied or stored in a manner that will result in the adulteration of product or the creation of insanitary conditions.
