Violation: 416.1, 416.2(b)(3), 416.4(b)

Citation: While walking through the Box Room I found the following noncompliances.The floor was very dirty, with a build up of dirt about 1/8th inch thick. there were holes in the walls, the possibility for rodents & insects to enter. The box conveyor is in bad need of repair, being held together with duck tape ,also a build up of dirt & residue on the track, also old equipment stored in the corners of the room, fat and meat particles inside the entrance doorway from the Cut-up department. Above the door going into the chemical room there is a large hole ,with what appears to be a bird/ rodent nest in this area. , holes in the walls, in the sanitation changing room you can see daylight under the garage door, potential for rodent/ insects to enter the building, was notified of these noncompliance's Establishment management has received oral & written communications concerning these noncompliances.

Regulation:

416.1 Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.

416.2(b)(3) Walls, floors, ceilings, doors, windows, and other outside openings must be constructed and maintained to prevent the entrance of vermin, such as flies, rats, and mice.

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
