Citation: At approximately 0810 hours, while performing an 04C04 ISP Code procedure for Finished Product Standards (Processing/Trim Nonconformance) on the Evisceration Line, the test exceeded the subgroup absolute limit of 30. My test results subgroup total was 31, the Processing Nonconformance's were two - extraneous material <, one - extraneous material >1", three - oil gland-whole, four - crop -partial-with mucosa, one-crop-whole, two - trachea>1", two - feathers-small, and two-feathers-large. I immediately notified (Quality Assurance) of my findings. (Evisceration Lead) was also shown the Processing nonconformance's. A check of the past five CUSUM's shows two CUSUM's exceed the start number eleven with a twenty and eighteen. I notified Production Manager and the IIC (Dr. of the failure mode. Quality Assurance immediately conducted a retest at the post chill location using prechill criteria for Processing Nonconformance, and passed at approximately 0821 hours. Quality Assurance performed two consecutive retests for Finished Product Standard processing nonconformance's at the Prechill Station at approximately 0829 hours and 0839 hours with both retests passing. Quality Assurance tagged 2 carcasses with tags to mark the end of the failure mode at approximately 0842 hours. At approximately 0853 hours and 0900 hours, Quality Assurance conducted retests at the post chill location using prechill criteria for the processing nonconformance, and both tests exceeded the tolerance number of 25, with a total of [redacted] At the Post Chill Station, Quality Assurance performed retests for processing nonconformance at approximately 0906 hours and 0912 hours, and passed. At approximately 0932 hours, the tags came out of the chiller marking the end of the failure mode. USDA and Quality Assurance retained four combo bins of whole carcasses (approximately 1000 carcasses), and fifteen boxes (40 lbs) of whole carcasses (approximately 600 pound) with USDA Retained Tag B39 133560, and Quality Assurance Retained Tags 9991, 9992, 9953, 9954, 9988, and 9989 for rework for lot #2.. This is a failure to comply with 9 CFR 381.76(b). This document serves as a written notification that your failure to comply with regulatory requirement could result in additional enforcement action described in 9 CFR part 500. Past Similar NRs - Previous Ineffective Plant Actions: Establishment's preventive measure "An additional employee will be placed on the line to address crop and or oil glands until the end of the lot upon notification of high FPS scores containing these defects from QA or USDA". The previous preventive measures have either not been implemented or were ineffective too prevent the recurrence of this noncompliance. NR: 61-2011 dated 8/10/2011
381.76(b) Presentation: Each inspector shall be flanked by an establishment employee assigned to be the inspector’s helper. The one inspector on the SIS–1 line shall be presented every bird. Each inspector on the SIS–2 line shall be presented every other bird on the line. An establishment employee shall present each bird to the inspector properly eviscerated with the back side toward the inspector and the viscera uniformly trailing or leading. Each inspector shall inspect the inside, viscera, and outside of all birds presented.
