Example of Document Request for Arizona Stop Animal Exploitation Now 21 August 2006 Dr. Jonathan Fink, Vice President for Research Dr. Fink, This request for records is made pursuant to section 39-121 et. seq.,
of the Statutes of Arizona. The documents requested from the Arizona State University are: all
health care records for primates from 6/29/04 – present. This will
include all daily care logs and necropsy reports for primates. Also
requested are copies of all currently active research protocols
involving primates. If these documents are available in electronic
format they are requested to be provided in that format. Otherwise
please provide hard copies. The requester is prepared to pay for all direct costs of duplication
of said records not to exceed an initial amount of one hundred dollars
($100.00). If the fees will exceed this amount provide me with a
detailed estimation of the fee, in advance before performing said work. The requester asserts that all records requested above are entitled
to be given to him as they are public records. Should you make an
opposite decision, please segregate all claimed exempt portions, provide
access to the remaining information, and provide requester with a
written detailed statement of your position with specific reference to
statutes of Arizona which you believe exempt this information from
release. Please respond to the above request within ten (10) working days of
